Paragon Physio (operated by Pent Group Pty Ltd, ABN 72 645 391 975, trading as Paragon Physio) is committed to protecting your privacy. This policy explains what information we collect, why we collect it, how we use it, and your rights.

What we collect

Identifying information

Name, date of birth, address, phone, email, emergency contact, GP details, Medicare or private health fund details, and NDIS plan information where relevant.

Health information

Clinical history, presenting condition, examination findings, treatment notes, P-PAR test data (VALD ForceFrame, HumanTrak, Dynamo, ForceDecks), progress reports, and discharge summaries.

Payment information

Card or bank details for processing payments, refunds, and recurring memberships. We use Stripe and other PCI-compliant payment processors; we do not store full card numbers on our systems.

Technical information

When you visit paragonphysio.com.au, we collect IP address, browser type, device type, pages visited, and referrer information for analytics and security.

How we use your information

• Delivering physiotherapy, exercise physiology, and rehabilitation services to you
• Communicating with you about appointments, treatment, and follow-ups
• Processing payments and managing your account
• Billing Medicare, NDIS, DVA, private health funds, and third-party insurers on your behalf
• Sending appointment reminders and (with consent) marketing communications
• Internal quality assurance, case reviews, and clinical audit
• Complying with our legal and professional obligations (AHPRA, Medicare, etc)

Who we share with

We may share your health information with:

• Other treating practitioners (with your consent) including GPs, specialists, surgeons, and other allied health providers
• Your NDIS support coordinator or plan manager where you are an NDIS participant
• Medicare, DVA, your private health fund, or third-party insurer for billing purposes
• Software providers who help us run the practice (Cliniko, GoHighLevel, Stripe, Xero, VALD) under strict confidentiality and data-protection agreements
• Legal authorities where required by law (subpoena, court order, mandatory reporting)

We never sell your personal or health information.

Marketing communications

If you opt in, we send periodic emails or SMS about new services, programs, events, and health tips. You can unsubscribe at any time using the link in any email or by replying STOP to any SMS. Opting out of marketing does not affect appointment-related communications.

Storage and security

Your information is stored in Cliniko (our practice management system, AWS Australian region), GoHighLevel (CRM), and Microsoft 365 (email). All systems are encrypted in transit and at rest. Access is restricted to authorised Paragon team members on a need-to-know basis.

Your rights

• Access – you can request a copy of your records at any time. We may charge a reasonable fee for large file extracts.
• Correction – if any information we hold is inaccurate, tell us and we'll correct it.
• Withdraw consent – you can withdraw consent for marketing, third-party sharing (where not legally required), or specific uses at any time.
• Complain – to us in the first instance via the contact below. If unresolved, you can contact the Office of the Australian Information Commissioner (oaic.gov.au).

Cookies and analytics

Our website uses cookies for session management, analytics (Google Analytics), and marketing pixels (Meta, Google Ads) where applicable. You can disable cookies in your browser settings – some site features may not work without them.

Children's privacy

Where we treat patients under 16, consent is obtained from a parent or legal guardian, and clinical records are shared with the consenting adult per AHPRA guidelines.

Changes to this policy

We may update this policy from time to time. Material changes will be notified via email to active patients. The current version always lives at paragonphysio.com.au/privacy.
